1. Two forms of Identification

• One government issued Photo ID; driver’s license, state ID, or passport
• Secondary ID with matching legal name as initial form of ID such as: vehicle registration card, Voter ID, Utility bill with same address as listed on ID

2. Your ORI Number and OCA Number (if applicable) 

• **If your background check is for AHCA, DCF, DOH, or EDOEA an OCA number is also required

3. Your Social Security Number (SSN card is not required)

• The “ORI” number is the Originating Agency Identifier, this number identifies to the FDLE Federal Department of Law enforcement who the requesting agency/ employer is and the purpose for the Background check. This number ensures your information if sent to the right place.
• This number is provided by the requesting state Agency such as: DBPR, AHCA, DCF, DOH.

• The OCA is the Originating Case Agency number and is required if your background check is for AHCA, DCF, DOH, or EDOEA.
• Your employer will provide you with a form that has all the required information on it- commonly these are referred to as a “Live Scan Request Form”. If you have not been provided this information, please request it from your manager/HR department.

• During your appointment we will provide you with what is called a “TCN “or Transaction Control Number.
